The eligibility for Long Term Care is based on an individual's need for assistance with activities of daily living, such as dressing, bathing, feeding, and toileting. Confusion and dementia can also be eligibility factors.To start the application process, call the Erie St Clair Home and Community Care Support Services (HCCSS), formerly the Erie St Clair LHIN, at 1-888-447-4468 to schedule a health assessment. Once your application is complete, they will notify you of your eligibility status and send your application to our Home. If our Home meets your needs, your name will be placed on our waitlist.Wait times can range from days, months, or even longer depending on the type of accommodation you're waiting for. The position of your name on our watilist can vary depending on the urgency of your need. Our Home is owned by a group of eight local Mennonite churches in our community and members of those churches are given priority on our waitlist by the HCCSS. Please note that we do not have access to information regarding our waitlist or the position of your name on our waitlist.
